What Is Tasimelteon?
Tasimelteon (Hetlioz) is an oral medication that treats specific sleep disorders. The sleep disorders treated by tasimelteon occur when there is a problem with your body’s internal clock (circadian rhythm or sleep-wake cycle).
This drug was first approved by the U.S. Food and Drug Administration in 2014 to treat Non-24-Hour Sleep-Wake Disorder in adults. It is also approved to treat nighttime sleep disturbances associated with Smith-Magenis Syndrome (SMS).
Hetlioz is available as a capsule for adults with Non-24 and patients aged 16 and older with SMS. Hetlioz LQ oral suspension is available for children ages 3 to 15 with SMS. Common side effects like headache, nightmares, or unusual dreams can occur while taking this drug.
Mechanism of Action (MOA)
To understand how tasimelteon works, let’s first briefly review what Non-24-Hour Sleep-Wake Disorder (Non-24) is.
Non-24-Hour Sleep-Wake Disorder (Non-24)
Non-24 is a serious, chronic condition that commonly affects people who are totally blind. Because they cannot perceive light, their brains do not receive the signals needed to synchronize the body’s internal clock with the normal 24-hour day.
Generally, your body has an internal clock, also called the sleep-wake cycle or circadian rhythm. It functions as your body’s natural clock, helping regulate when you feel awake and when you feel sleepy over a roughly 24-hour cycle. In people who can see, sunlight helps reset this clock every day.
When light reaches the brain during the day, it helps keep you awake and alert. As it gets dark, your brain produces more melatonin, a hormone that helps regulate your sleep-wake cycle. The increase in melatonin signals to your body that it is time to sleep.
People who are totally blind (Non-24) do not receive these light signals. As a result, their internal clock slowly falls out of sync with the normal 24-hour day. Their bedtime keeps shifting later, which makes it hard to sleep at night and causes excessive daytime sleepiness.
Hetlioz helps synchronize the body’s internal clock with the 24-hour day, improving nighttime sleep and daytime alertness in people with Non-24.
Hetlioz Working Mechanism
Tasimelteon is a melatonin receptor agonist, which is a class of medication designed to mimic the action of the melatonin hormone. Tasimelteon activates the MT1 and MT2 melatonin receptors in the brain, mimicking the effects of natural melatonin. By helping synchronize the body’s internal clock with the 24-hour day, it improves nighttime sleep and daytime alertness in people with Non-24-Hour Sleep-Wake Disorder and helps treat nighttime sleep disturbances associated with Smith-Magenis Syndrome.

Recommended Dosage
Dosage for Non-24-Hour Sleep-Wake Disorder
The recommended dosage of Hetlioz for adults with Non-24-Hour Sleep-Wake Disorder is 20 mg (1 capsule) taken one hour before bedtime on an empty stomach.
Dosage for Nighttime Sleep Disturbances in SMS
For nighttime sleep disturbances associated with Smith-Magenis Syndrome (SMS), the dose of Hetlioz is based on the patient’s age and body weight.
Usual Adult Dose
For patients (16 years of age or older), the recommended dose is also 20 mg once daily, taken 1 hour before bedtime on an empty stomach.
Pediatric Dose for Children Ages 3 to 15
For children ages 3 to 15 with Smith-Magenis Syndrome, Hetlioz is given as a 4 mg/ml oral suspension. The dose is based on body weight:
- 28 kg (about 62 lb) or less: 0.7 mg/kg is given once a day on an empty stomach before bedtime.
- More than 28 kg: 20 mg (5 ml of the 4 mg/ml oral suspension) once daily, one hour before bedtime, without food.
Tasimelteon Side Effects
Common side effects include:
- Headache
- Nightmares or strange dreams
- Increased liver enzyme (ALT) levels
- Upper respiratory or urinary tract infections
- Nausea
- Dizziness
