What Is Mifepristone 300 mg (Korlym)?

Korlym is a brand-name prescription medication. The active ingredient in this product is mifepristone, which belongs to a drug class called cortisol receptor blockers. The FDA approved Korlym in 2012 to control hyperglycemia secondary to endogenous Cushing’s syndrome in adults with type 2 diabetes or glucose intolerance who are not candidates for surgery or whose surgery has not been successful.  

Cushing’s syndrome results from prolonged exposure to elevated cortisol levels. Cortisol helps regulate blood sugar, metabolism, blood pressure, and the body’s response to stress. When cortisol levels remain too high over time, they can increase blood sugar levels and contribute to type 2 diabetes or glucose intolerance. 

Korlym may be prescribed to control high blood sugar in adults when they:

  • Have endogenous Cushing’s syndrome (too high cortisol levels usually due to  tumors or enlarged growth on adrenal or pituitary glands)
  • Have type 2 diabetes or glucose intolerance 
  • Cannot have surgery or if surgery is ineffective

This medication comes as light yellow to yellow, oval-shaped tablets to take by mouth. Each tablet contains 300 mg of mifepristone. 

Korlym is not recommended for use in patients with type 2 diabetes originating from Cushing’s syndrome.

Mechanism of Action

Mifepristone lowers cortisol activity by preventing cortisol from binding to specific sites (cortisol receptors) throughout the body. That way, it helps control blood sugar levels.  

Dosing 

The typical starting dose is 300 mg once daily, administered orally with a meal. Depending on how your body responds, your healthcare provider may increase the dose to a maximum of 1,200 mg per day. However, the daily dose shouldn’t exceed 20 mg/kg.

Mifepristone 300 mg and Weight Loss

Weight loss (more than or equal to 5% of body weight) has been observed in some studies involving patients with endogenous Cushing’s syndrome who received Korlym. However, it is unclear whether the weight loss is due solely to Korlym or to improvements in Cushing’s syndrome during treatment. Most importantly, Korlym is not an anti-obesity medication.

Proper Use and Storage

  • Take your doses exactly as directed. Take your medicine around the same time each day.
  • Doses may be adjusted if you have kidney or liver problems. 
  • Swallow the tablets whole. Do not crush, chew, or split them. If you have difficulty swallowing tablets, talk to your healthcare provider or pharmacist. 
  • Don’t drink grapefruit juice while you take the tablets. 
  • If you miss a dose, take the missed dose immediately after you remember. Skip the missed dose if it is time for the next scheduled dose. Never take a double dose to compensate for the missed one. 
  • If you think an overdose has occurred, call the poison control helpline at 1-800-222-1222 immediately. 
  • Store this medication at temperatures between 68°F and 77°F (20°C and 25°C).
  • Don’t store your medicines in the bathroom. 
  • Keep all medications out of reach of children and pets.

Mifepristone Side Effects

Common side effects include:
  • Nausea
  • Vomiting
  • Headache
  • Pain in the arms and legs
  • Swelling of the arms and legs 
  • Dizziness
  • Thickening of the uterine lining
  • Tiredness
  • Low blood level of potassium
  • Elevated blood pressure
  • Decreased appetite
Severe side effects include:

Signs of adrenal insufficiency

  • Severe tiredness or weakness
  • Nausea or vomiting
  • Low blood pressure
  • Dizziness or fainting

Signs of low potassium

  • Muscle weakness or cramps
  • Rapid or irregular heartbeat

Other serious symptoms

  • Low blood sugar with sweating, shakiness, or irritability
  • Unexpected vaginal bleeding
  • Shortness of breath
