Imiquimod 3.75% (Zyclara) Introduction and Uses

Zyclara is a brand-name prescription medication. The active ingredient in this product is imiquimod, which is in a drug class called immune response modifiers

Imiquimod 3.75% cream (for topical use) is FDA-approved to treat: 

  • Actinic keratosis (scaly patches on the skin due to long-term sun exposure) of the face and balding scalp in adults with healthy immune systems
  • Warts on the outside of the genital and around the anal areas in individuals at least 12 years old who have healthy immune systems 

This medication is available as a topical cream supplied in pump bottles or single-use packets.

Imiquimod 3.75% (Zyclara) Mechanism of Action 

Although its exact mechanism is not fully understood, imiquimod activates the body’s immune system by stimulating the production of cytokines, including interferon-alpha and other immune signaling proteins, which help the body eliminate abnormal or virus-infected skin cells. Imiquimod does not kill viral cells directly.

Before Using Imiquimod 3.75% (Zyclara)

Before using the cream, inform your healthcare provider if you:

  • Are allergic to this product or any product component
  • Have a weakened immune system, an autoimmune disorder, or chronic graft-versus-host disease
  • Have other skin issues or sunburn
  • Are pregnant or plan to get pregnant 
  • Are breastfeeding or plan to breastfeed (Ask your healthcare provider for instructions on feeding a baby during treatment with medication)

Imiquimod 3.75% (Zyclara) Dosing 

Actinic keratosis

Apply a thin layer once daily at bedtime to the affected skin on your face or balding scalp for 14 days. Then, stop treatment for 14 days. Resume application after 14 days for the next 14 days. Don’t use more than 0.5 grams per application, and rub the cream in completely until it is no longer visible on the skin. 

The treated skin may appear worse before it improves, and complete healing may not be visible until several weeks after treatment is finished. 

External genital warts

Apply a thin layer once daily at bedtime until the warts go away or for up to 8 weeks. Don’t use more than 0.25 grams at each application.

Imiquimod 3.75% Side Effects

Common side effects include:
  • Skin redness, irritation, scabbing, crusting, flaking, scaling, dryness, swelling
  • Headache
  • Irritation or pain in the treated area
  • Tiredness
  • Nausea

 

Severe side effects include:
  • Broken skin or weeping sores, especially during the first few days of treatment
  • Flu-like symptoms such as nausea, fever, chills, tiredness, and muscle weakness or pain
  • Swelling of the area outside the vagina that may cause problems with urination
