When Is Infusion Therapy Recommended for Lupus?

Infusion therapy may be recommended for patients with lupus when standard oral medications are not enough to adequately control symptoms or prevent disease progression. FDA-Approved Infusion Therapies for Lupus

Gazyva® (obinutuzumab) — For Lupus Nephritis

Gazyva® (obinutuzumab) is an FDA-approved infusion therapy for adults with active lupus nephritis who are receiving standard treatment, including mycophenolate mofetil and corticosteroids. Approved in October 2025, it represents one of the few targeted therapies specifically indicated for lupus nephritis.

Obinutuzumab is a CD20-directed monoclonal antibody that works by targeting and depleting B cells, which play a key role in driving the autoimmune inflammation associated with lupus. By reducing these disease-causing cells, Gazyva® helps control kidney inflammation and may help prevent or delay progression to advanced kidney disease.

The medication is administered intravenously on a structured schedule: an initial 1,000 mg infusion, followed by a second dose at week 2, then additional doses at weeks 24 and 26, and every 6 months thereafter. After the first infusion, eligible patients may receive subsequent infusions over a shorter duration of approximately 90 minutes.

IV Corticosteroids (Pulse Therapy)

Intravenous (IV) corticosteroids, such as methylprednisolone, are commonly used as pulse therapy to quickly control severe lupus flares or rapidly reduce inflammation in active lupus nephritis.

These high-dose treatments are typically given over a short period to stabilize the condition and prevent further organ damage. IV corticosteroids are generally used as part of an overall treatment plan and are not intended for long-term use on their own due to potential side effects.

IVIG (Off-Label)

Intravenous immunoglobulin (IVIG) may be considered in select cases of lupus, particularly when the disease is refractory to standard treatments, in severe lupus nephritis, or when certain hematologic complications are present.

Although not specifically FDA-approved for lupus, IVIG is used off-label to help regulate the immune system. It works by providing a concentrated pool of normal antibodies that can help counteract the abnormal autoimmune activity seen in lupus, supporting better disease control in complex cases.
