What Is Trazimera and How Does It Work?
Trazimera is a medication with the generic name trastuzumab-qyyp. It is a biosimilar medication to trastuzumab and belongs to the class of medications known as monoclonal antibodies. Trastuzumab was first approved by the FDA on September 25, 1998, for the treatment of patients with metastatic breast cancer whose tumors overexpress the HER-2 protein and who have received one or more chemotherapy regimens for their metastatic disease. It was also approved to be used in combination with another chemotherapy medication called paclitaxel for the treatment of patients with metastatic breast cancer whose tumors overexpress HER-2 protein and who have not received chemotherapy for their metastatic disease. Trazimera was approved as a biosimilar to trastuzumab on March 11, 2019.
Trazimera is a monoclonal antibody that binds to the extracellular domain of the human epidermal growth factor receptor 2 protein (HER-2). By doing so, it triggers cell destruction of those cancerous cells that are expressing the HER-2 protein. This is known as antibody-dependent cellular cytotoxicity, and it inhibits the rapid increase in the number of cells that overexpress HER-2 protein (cancerous cells).
What Is Trazimera Used To Treat?
Trazimera is FDA-approved for the treatment of:
- Breast cancer as an adjuvant treatment: Adjuvant treatment of HER2-overexpressing breast cancer, as part of a treatment regimen consisting of cyclophosphamide, doxorubicin, and either docetaxel or paclitaxel, or as part of a treatment regimen with docetaxel and carboplatin, or as a single agent following multimodality anthracycline-based therapy.
- Metastatic Breast Cancer:
- First-line treatment of HER2-overexpressing metastatic breast cancer (in combination with paclitaxel).
- As a single-agent treatment of HER2-overexpressing breast cancer in patients who have previously received one or more chemotherapy regimens for metastatic disease.
- Metastatic Gastric Cancer: Treatment of HER2-overexpressing metastatic gastric or gastroesophageal junction adenocarcinoma (in combination with cisplatin and either capecitabine or 5-fluorouracil) in patients who have not received prior treatment for metastatic disease.
Patients should be selected for breast and gastric cancer therapy based on an approved companion diagnostic test for tumor specimens for HER2 overexpression, or HER2 gene amplification.

Trazimera Dosing Information
Trazimera is available as a vial for reconstitution and intravenous infusion. The available vial sizes are as follows:
- 150 mg vial
- 420 mg vial
Your Trazimera dose depends on your body weight and the indication you’re being treated for:
- Trazimera is usually given as a higher loading dose (4 – 8 mg/kg) followed by a lower maintenance dose (2 – 6 mg/kg).
- The dose is then diluted in normal saline and is then infused through a catheter that gets inserted into a vein in the arm.
- Infusions usually take about 90 minutes followed by a period of observation for any infusion-related reactions. Maintenance doses may sometimes be infused over shorter periods of time depending on prior infusions being well-tolerated.
Patients who receive Trazimera need to be observed closely during the infusion for fever, chills, or other infusion-related symptoms. Treatment with acetaminophen, diphenhydramine, or meperidine is usually effective for managing infusion-related events.
