Severe AIHA can be life threatening.&nbsp;<\/p>\n\n\n\n<p>Estimates suggest 1 to 3 cases per 100,000 people each year [<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11452171\/\">1<\/a>].<\/p>\n\n\n\n<p>Individuals with this condition don\u2019t have enough red blood cells (RBCs) in their blood. AIHA occurs when the immune system fails to recognize RBCs and attacks them, leading to their early death.&nbsp;<\/p>\n\n\n\n<p>RBCs stay in the body for about 120 days. However, autoimmune hemolytic anemia causes the body to remove RBCs within a few days.&nbsp;<\/p>\n\n\n\n<p>Anyone, irrespective of their age or sex, can develop AIHA. However, it\u2019s more common in women. In about half of the cases, the exact cause is unknown (idiopathic autoimmune hemolytic anemia). In others, it may be caused by lupus, lymphoma, or certain medicines.&nbsp;<\/p>\n\n\n\n<p>The two main types are:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-1-warm-aiha\"><span class=\"ez-toc-section\" id=\"1_Warm_AIHA\"><\/span>1. Warm AIHA\u00a0<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>In this type, antibodies (proteins directed against RBCs) react with red blood cells at normal body temperature, 98.6\u00b0F. This type of AIHA is more common, accounting for about 90% of all cases.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-2-cold-aiha\"><span class=\"ez-toc-section\" id=\"2_Cold_AIHA\"><\/span>2. Cold AIHA<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>Cold AIHA is also known as cold agglutinin disease (CAD). In this type, cold-agglutinin antibodies attack red blood cells at temperatures between 32\u00b0 and 39.2\u00b0F.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-autoimmune-hemolytic-anemia-treatment-options-where-does-ivig-fit\"><span class=\"ez-toc-section\" id=\"Autoimmune_Hemolytic_Anemia_Treatment_Options_Where_Does_IVIG_Fit\"><\/span>Autoimmune Hemolytic Anemia Treatment Options: Where Does IVIG Fit?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>There\u2019s a lack of clearly defined treatment guidelines for managing autoimmune hemolytic anemia. Available treatment options rely primarily on small studies, case reports, and expert experiences and recommendations.<\/p>\n\n\n\n<p>The first line of treatment for AIHA is steroids (along or with IVIG).&nbsp;<\/p>\n\n\n\n<p>Steroids and RBC transfusion are the first-line treatment for warm autoimmune hemolytic anemia. Steroids rarely benefit people with cold AIHA. First-line treatment for cold AIHA is a monoclonal antibody such as <a href=\"https:\/\/ameripharmaspecialty.com\/cancer\/rituximab-rituxan\/\">rituximab<\/a>. Experts may also recommend high-dose IVIG and <a href=\"https:\/\/ameripharmaspecialty.com\/ivig\/ivig-vs-plasmapheresis\/\">plasmapheresis<\/a>; however, results may be unpredictable.&nbsp;<\/p>\n\n\n\n<p>Splenectomy (spleen removal surgery) may also be a viable recommendation, provided the patient is eligible for surgery. If surgery isn\u2019t possible or the patient relapses after spleen removal, IVIG may be considered.&nbsp;<\/p>\n\n\n\n<p>IVIG may be used \u201coff-label\u201d for AIHA. Off-label drug use is when a healthcare provider prescribes a drug for conditions other than those mentioned in the prescribing information for the medication.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-ivig-for-autoimmune-hemolytic-anemia-when-it-may-be-an-option\"><span class=\"ez-toc-section\" id=\"IVIG_for_Autoimmune_Hemolytic_Anemia_When_It_May_Be_an_Option\"><\/span>IVIG for Autoimmune Hemolytic Anemia: When It May Be an Option<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>IVIG for autoimmune hemolytic anemia may be an option in the following situations [<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2468124524000640\">2<\/a>]:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Recurrent or chronic (long-term) AIHA.<\/li>\n\n\n\n<li>When steroids alone don\u2019t work, IVIG may be used as a second-line or adjunctive therapy.\u00a0<\/li>\n\n\n\n<li>Your hematologist (blood disorder specialist) may consider IVIG for autoimmune hemolytic anemia in children who don\u2019t respond to steroids, are steroid-dependent, or relapse following the initial response.\u00a0<\/li>\n<\/ul>\n\n\n\n<p>The \u201cGuidelines on the Use of Intravenous Immune Globulin for Hematologic Conditions\u201d don\u2019t recommend routine use of IVIG for acute or chronic treatment of AIHA. IVIG may help your body remove the RBC-destroying antibodies.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-ivig-for-autoimmune-hemolytic-anemia-how-effective-is-this-therapy\"><span class=\"ez-toc-section\" id=\"IVIG_for_Autoimmune_Hemolytic_Anemia_How_Effective_Is_This_Therapy\"><\/span>IVIG for Autoimmune Hemolytic Anemia: How Effective Is This Therapy?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Results are mixed and conflicting.&nbsp;<\/p>\n\n\n\n<p>For example, according to researchers from the Department of Pediatrics, Division of Hematology\/Oncology at the Baylor College of Medicine, IVIG is ineffective in AIHA [<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9821124\/\">6<\/a>].<\/p>\n\n\n\n<p>Likewise, researchers of a 2015 study say that IVIG doesn\u2019t improve treatment outcomes in individuals with AIHA. Also, they found no difference in relapse rates between participants who received IVIG and those who didn\u2019t receive IVIG [<a href=\"https:\/\/journals.lww.com\/jpho-online\/fulltext\/2016\/03000\/clinical_features_and_treatment_outcomes_of.22.aspx\">7<\/a>].<\/p>\n\n\n\n<p>On the other hand, according to a 2024 study, IVIG (in addition to standard of care) may be used as a rescue therapy in adults with severe AIHA [<a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/ajh.27361\">8<\/a>].<\/p>\n\n\n\n<p>Likewise, IVIG (0.4\u20130.5 g\/kg for 5 days) may benefit children with AIHA [<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2468124524000640\">2<\/a>].<br>In a 2019 case report, a 25-year-old African American woman was successfully treated with prednisone, IVIG, and <a href=\"https:\/\/ameripharmaspecialty.com\/cancer\/velcade-bortezomib-a-promising-treatment-for-multiple-myeloma\/\">bortezomib<\/a>. According to the researchers, she has been in remission (a period of a decrease or disappearance of symptoms) since the completion of therapy [<a href=\"https:\/\/www.ijbti.com\/\/archive\/article-full-text\/100046Z02IC2019\">9<\/a>].<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-ivig-may-be-considered-in-emergencies\"><span class=\"ez-toc-section\" id=\"IVIG_May_Be_Considered_in_Emergencies\"><\/span>IVIG May Be Considered in Emergencies<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>IVIG can be an option for emergencies, but there\u2019s little solid evidence to support its use.
