Necrotizing myopathy, also known as necrotizing autoimmune myopathy (NAM) or immune-mediated necrotizing myopathy (IMNM), is characterized by cell death and necrosis of muscle cells, particularly the muscles closer to the center of the body. This causes generalized muscle weakness and difficulty moving. It is considered a very rare form of idiopathic inflammatory myopathy, in which muscle biopsy typically shows prominent muscle fiber necrosis with minimal inflammatory cell infiltration. Although classified as an inflammatory myopathy, necrosis predominates over visible inflammation on histopathology. This particular form is quite rare, but the incidence is increasing due to the increased rate of detection. Currently, the incidence rate of all idiopathic inflammatory myopathies is about 1.16 to 19 cases per million people per year, and necrotizing myopathy represents approximately 10% of these cases [<a class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p><span style=\"font-weight: 400;\">The exact cause is poorly understood, but necrotizing myopathy is thought to be an autoimmune disorder. In an autoimmune disorder, a certain trigger stimulates the body to attack its own cells and tissues. The triggers that can lead to necrotizing autoimmune myopathy include:<\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><b>Statins:<\/b><span style=\"font-weight: 400;\"> A group of drugs used to control the level of cholesterol in the body.<\/span><\/li>\n\n\n\n<li><b>Cancer:<\/b><span style=\"font-weight: 400;\"> Certain types of cancers are associated with necrotizing myopathy.&nbsp;<\/span><\/li>\n\n\n\n<li><b>Connective tissue disease:<\/b><span style=\"font-weight: 400;\"> This is a disease that affects the connective tissue (the tissue that connects all the parts of the body together). Necrotizing myopathy may overlap with other autoimmune connective tissue diseases due to shared immune-mediated mechanisms, rather than because of the structural composition of muscle tissue.<\/span><\/li>\n\n\n\n<li><b>HIV:<\/b><span style=\"font-weight: 400;\"> HIV is a virus that reduces the immune response of the body to almost every foreign object, germ, or bacteria. In some cases, the body reacts to HIV infection by attacking its own cells, including muscle cells, causing myopathy.<\/span><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Signs_And_Symptoms_Of_Myopathy\"><\/span><span style=\"font-weight: 400;\">Signs And Symptoms Of Myopathy&nbsp;<\/span><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p><span style=\"font-weight: 400;\">Almost all types of myopathy present with similar symptoms, making it difficult to distinguish the type of myopathy without further testing.&nbsp;<\/span><span data-preserver-spaces=\"true\">Patients who have necrotizing myopathy may experience symptoms such as the following:<\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><span data-preserver-spaces=\"true\">Weakness in the muscles closest to the middle of the body, such as the shoulders, forearms, thighs, hips, neck, and back.<\/span><\/li>\n\n\n\n<li><span data-preserver-spaces=\"true\">Difficulty standing up from a chair.<\/span><\/li>\n\n\n\n<li>Difficulty <span data-preserver-spaces=\"true\">climbing stairs.<\/span><\/li>\n\n\n\n<li><span data-preserver-spaces=\"true\">Falling or difficulty getting up from a fall.<\/span><\/li>\n\n\n\n<li><span data-preserver-spaces=\"true\">Difficulty raising arms over the head.<\/span><\/li>\n\n\n\n<li><span data-preserver-spaces=\"true\">A general feeling of tiredness.<\/span><\/li>\n<\/ul>\n\n\n\n<p><span style=\"font-weight: 400;\">Other muscles may be affected as well, although far less commonly. These include:<\/span><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><span style=\"font-weight: 400;\">Muscles of the neck, causing difficulty in head and neck movement.<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400;\">Muscles of the throat, causing difficulty swallowing or voice changes.<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400;\">Chest muscles, causing difficulty breathing.<\/span><\/li>\n\n\n\n<li><span style=\"font-weight: 400;\">Muscles of the heart.<\/span><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"How_Can_Immune-Mediated_Necrotizing_Myopathy_Be_Diagnosed\"><\/span><span style=\"font-weight: 400;\">How Can Immune-Mediated Necrotizing Myopathy Be Diagnosed?<\/span><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><b><a href=\"https:\/\/jnnp.bmj.com\/content\/76\/suppl_2\/ii32\">Electromyography<\/a>: <\/b><span style=\"font-weight: 400;\">This is an electrical test of the muscles to check their strength. These tests usually show myopathic symptoms (meaning symptoms of weakness) but are not particularly indicative of the cause or type of myopathy.<\/span><\/li>\n\n\n\n<li><b>Blood markers: <\/b>Blood tests show autoimmune markers, mainly SRP and HMGCR autoantibodies. These are autoimmune antibodies produced when the body is exposed to the above-mentioned triggers and are possibly the main cause. Highly elevated levels (up to 10 times the normal levels) of creatine kinase enzyme can also be seen. However, seronegative findings (meaning no change in the blood markers and antibodies) are<span style=\"font-weight: 400;\"> common in 20% of patients, so blood tests should not be a definitive determinant of the absence of the disease.<\/span><\/li>\n\n\n\n<li><b>MRI: <\/b><span style=\"font-weight: 400;\">Magnetic resonance imaging (MRI) is a commonly used test to check the condition of soft tissues. With necrotizing myopathy, muscles show signs of edema.&nbsp;<\/span><\/li>\n\n\n\n<li><b>Muscle Biopsy: <\/b><span style=\"font-weight: 400;\">This is the most definitive test of necrotizing myopathy, where a small sample is taken and analyzed.\u00a0 It is the only test that can differentiate it from other idiopathic inflammatory myopathies (IM) or <a href=\"https:\/\/ameripharmaspecialty.com\/other-health-conditions\/polymyositis\/\">polymyositis<\/a> (PM), whose signs and symptoms are nearly indistinguishable from one another. Unlike polymyositis and other inflammatory myopathies, immune-mediated necrotizing myopathy shows prominent muscle fiber necrosis with relatively sparse inflammatory infiltrates on biopsy. Each has different treatments and risk factors. The categories are based on the presence of different autoantibodies found in the blood.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Patients_with_SRP_Autoantibodies\"><\/span>Patients with SRP Autoantibodies<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4440264\/#:~:text=Anti%2Dsignal%20recognition%20particle%20(SRP,antibodies%20seems%20to%20be%20broad.\">SRP autoantibodies<\/a> appear in blood tests of many people who have symptoms of necrotizing myopathy. Sudden and extreme muscle weakness can occur, and there may also be cardiac involvement.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Patients_with_HMGCR_Autoantibodies\"><\/span>Patients with HMGCR Autoantibodies<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>HMGCR is a key enzyme in the production of cholesterol. Patients who have <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27618830\/#:~:text=Anti%2DHMGCR%20antibodies%20represent%20a,with%20other%20autoimmune%20rheumatic%20diseases.\">HMGCR autoantibodies<\/a> and use statin medications for high cholesterol may develop this kind of statin-induced necrotizing myopathy.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"All_Other_Patients\"><\/span>All Other Patients<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>All other patients are classified by some experts in a third, separate group of necrotizing myopathy. These patients do not have SRP or HMGCR autoantibodies.<\/p>\n\n\n\n<p>Prognosis may vary between subtypes. Patients with anti-SRP antibodies often experience more severe and rapidly progressive weakness and may be more resistant to treatment. Patients with anti-HMGCR antibodies, particularly those with statin exposure, often respond well to immunotherapy, especially when diagnosed early. Early treatment is associated with better functional outcomes in all subtypes.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Is_Necrotizing_Autoimmune_Myopathy_Painful\"><\/span><span style=\"font-weight: 400;\">Is Necrotizing Autoimmune Myopathy Painful?<\/span><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p><span style=\"font-weight: 400;\">Since myopathy is a disease of the muscles, myalgia (muscle pain) is often an accompanying symptom. Muscle pain may result from ongoing muscle fiber damage, inflammatory mediators, and immune system activity within the muscle tissue. Pain may also develop due to muscle fatigue, deconditioning, and secondary strain from weakened muscle groups.<\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Can_You_Recover_From_Necrotizing_Myopathy\"><\/span><span style=\"font-weight: 400;\">Can You Recover From Necrotizing Myopathy?<\/span><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p><span style=\"font-weight: 400;\">There is no known cure for necrotizing myopathy. Since the problem lies within the body\u2019s immune system being sensitized to attack the body\u2019s cells, it is not possible to cure such a disease as you cannot turn off a patient\u2019s immune system. However, the condition can be controlled, and patients can lead a normal life in most cases.<\/span><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"What_Is_The_Treatment_For_Necrotizing_Myopathy\"><\/span><span style=\"font-weight: 400;\">What Is The Treatment For Necrotizing Myopathy?&nbsp;<\/span><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p><span style=\"font-weight: 400;\">Treatment of immune-mediated necrotizing myopathy is often a multifold approach:<\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Removal_of_the_Cause\"><\/span><span style=\"font-weight: 400;\">Removal of the Cause<\/span><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p><span style=\"font-weight: 400;\">While some cases have no apparent cause, most cases come with one of the associated triggers mentioned above. Removal of the cause ensures the problem does not progress. Patients on statin therapy must consult their physician to stop or change their drugs. Patients who have cancer should discuss the best options available to control both diseases simultaneously. Treatment for connective tissue disease and HIV infections usually involves corticosteroids.<\/span><\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Corticosteroids_Therapy\"><\/span><span style=\"font-weight: 400;\">Corticosteroids Therapy<\/span><span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p><span style=\"font-weight: 400;\">Corticosteroids are the main line of treatment. They suppress the immune response and reduce the production of inflammatory cytokines, helping to limit further muscle damage.
