What Is Zejula?

Zejula (niraparib) is an oral targeted therapy medication used for the treatment of ovarian, fallopian tube, and primary peritoneal cancer. Zejula is a PARP (poly adenosine diphosphate-ribose polymerase) inhibitor that specifically targets cancer cells and exerts less harm on healthy cells in the body. PARP is a protein found in human cells which aids in repairing damaged cells. PARP inhibitors as targeted treatments work by blocking the repair of cancerous cells and allowing them to die.

PARP inhibitors are given to patients after full or partial response to initial chemotherapy with a platinum-based medication. Zejula is used as a maintenance treatment to help prevent cancer from coming back. Zejula is a once-daily oral maintenance medication treatment available in 100 mg capsules that can be taken with or without food.

What is Zejula Used to Treat?

Zejula is FDA approved for adult patients as a maintenance treatment for recurrent maintenance therapy. It is approved for women with advanced ovarian cancer in complete or partial response, such as advanced or recurrent:

  • Epithelial ovarian cancer
  • Fallopian tube cancer
  • Primary peritoneal cancer

Zejula is also used for the treatment of adult patients who have been treated with three or more prior chemotherapy regimens associated with homologous recombination deficiency (HRD) positive status defined by either:

  • A harmful or suspected-to-be-harmful BRCA mutation, or
  • A genomic instability that progressed more than 6 months after response to the last chemotherapy session.

Zejula is only available in 100 mg capsules, and the recommended dosages are shown below:

First-line Maintenance Treatment for Advanced Ovarian Cancer
For patients who weigh < 77 kg (<170 lbs) OR with a platelet count <150,000/ μL, the recommended dose is 200 mg taken by mouth once a day. For patients who weigh >77 kg (>170 lbs) OR with a platelet count >150.000/ μL, the recommended dose is 300 mg taken by mouth once a day.

Maintenance Treatment for Recurrent Ovarian Cancer
The recommended dose is 300 mg (3 capsules) taken orally once daily.

Treatment of Advanced Ovarian Cancer After Three or More Chemotherapy Regimens
The recommended dose is 300 mg (3 capsules) taken orally once daily.
Zejula can be taken with or without food. Take the medication at approximately the same time each day. Taking the medication at bedtime may help relieve symptoms of nausea.

Treatment with Zejula will require routine monitoring of blood pressure, heart rate, and blood cell count.

Continue Zeluja treatment until disease progression or unacceptable toxicity.

It is not known if Zejula is safe and effective in children.

Zejula can cause fetal harm if administered to pregnant patients. Avoid breastfeeding and use effective contraceptives during treatment and for 6 months after the last dose of Zeluja. If you get pregnant, contact your doctor immediately.

Zejula Side Effects

Common side effects include:

  • Mouth sores
